Referrals are welcome

Who can make a referral?

A referral to OdysseyCare can come from the participant themselves, or from someone supporting them. Where someone else makes the referral, we confirm the participant’s consent, or the referrer’s authority, before we go any further.

A participant

You are welcome to refer yourself, ask questions first, or have someone you trust help you with the referral.

Family, carer or advocate

A family member, carer, nominee, advocate or other trusted person can contact us with the appropriate consent or authority.

A coordinator or professional

Support Coordinators, Psychosocial Recovery Coaches, LACs, GPs, allied health teams, hospitals and community partners can all refer.

A referral does not remove your choice and control

You can ask questions, compare providers, involve someone you trust and decide whether OdysseyCare is the right fit for you. Sending this form does not create an obligation to proceed — see the OdysseyCare promise for how we work with the people we support.

Before you begin

Information that helps us respond

You do not need every detail before contacting us. The following helps our Intake team understand the request and prepare for the first conversation.

  • The participant’s name, suburb or preferred service area, and safe contact details.
  • The support being requested, and their main goals or preferences.
  • Their NDIS funding category and how the plan is managed, if known.
  • The preferred start timeframe, and whether the request is time-sensitive.
  • Any communication, accessibility or cultural considerations for the first conversation.
  • Your relationship to the participant, and confirmation that you are authorised to make contact.

    This might be a meeting, an assessment, a home visit, or a service proposal — whatever suits the request.
